Insurance Policy

Your insurance policy is a contract between you and your insurance company. You are expected to understand your policy and contact them with specific questions. If there is a change in your insurance or coverage, it is your responsibility to notify our office. Our staff can answer general questions and provide you with ESTIMATES on your co-pay amount due. We are unable to guarantee payments or coverage. Once payment is received from your insurance company, you are responsible for any remaining balance.

As a service to our patients, we will bill your insurance. Any unpaid balances over 60 days will be the responsibility of the patient and the balance must be paid in full.
